If it is your first visit we recommend you arrive at least 10 minutes beforehand to fill out a registration form. You can download the form (click HERE), fill it in at home, and bring it with you on the day. Alternatively you may fill in the form when you attend your procedure. All information is strictly confidential, and is stored securely, in line with the Data Protection Act. All appointments are scheduled with each individual treatment in mind so 72 hour minimum notice for cancellation applies. Please note that the deposit is non-refundable.
